Your doctos can begin referring patients shortly after onboarding. During the appointment, your medical assistant enrolls the patient through your dedicated practice admin web portal.

The patient immediately receives a secure text message with onboarding instructions and a link to download the OptiBMIze mobile app on either the App Store/Google Play Store. Practices may choose to provide patients with a smart ceulluar weight scale at the end of the visit, or OptiBMIze can ship the device directly to the patient after enrollment.

Once the patient downloads the app, an OptiBMIze care coordinator connects with the patient to schedule an initial consultation. During this conversation, the care team learns about the patient’s current health status, goals, and challenges.

From there, a personalized support plan is created for the patient, and ongoing engagement, monitoring, and progress updates are managed between visits while keeping your practice informed.
