Once the patient downloads the app, an OptiBMIze care coordinator connects with the patient to schedule an initial consultation. During this conversation, the care team learns about the patient’s current health status, goals, and challenges.

From there, a personalized support plan is created for the patient, and ongoing engagement, monitoring, and progress updates are managed between visits while keeping your practice informed.
